The medium is a modified Sabouraud dextrose agar containing cycloheximide, gentamicin, and chlortetracycline to suppress saprophytic molds and bacteria, plus phenol red as a color indicator.

The color change is the point of the test. Dermatophytes preferentially metabolize protein rather than carbohydrate, and protein digestion releases alkaline byproducts that shift the phenol red indicator from amber to bright red. Most contaminant fungi consume the carbohydrate first, producing acid, and only turn the medium red days later once the sugar is exhausted. Visual diagnosis of ringworm is unreliable because dermatophytosis is a great clinical mimic, and no lesion pattern is specific to it. For animals without obvious lesions, for screening in a shelter or breeding cattery, or for post-treatment monitoring, the Mackenzie toothbrush technique is preferred. A new, sterile-packaged toothbrush is brushed vigorously over the entire coat for two to three minutes, with focused attention on the face, ears, and paws, then the bristles are pressed directly into the agar surface. Plates are incubated at 24 to 27 degrees Celsius with humidity, and examined daily rather than at the end, because the diagnostic signal is the timing relationship between colony growth and color change.

The reading rule is simple. A dermatophyte turns the medium red at the same time the colony first becomes visible, and the colony itself is pale: white, cream, buff, or lightly tan, with a cottony or powdery surface. A contaminant typically appears as a pigmented colony, green, black, or gray, and turns the medium red only later, after several more days of growth. A dark colony with red medium is a contaminant until proven otherwise.

Color change alone never confirms a diagnosis. Definitive identification requires microscopic examination of macroconidia, harvested with clear acetate tape and stained with lactophenol cotton blue. Several factors extend time to growth. Animals that have already started systemic or topical antifungal therapy, samples with low fungal burden, and refrigerated or delayed-transit samples all grow slower. This is why a 7 day negative means nothing, and why laboratories that report at 14 days generally note that a 21 day hold is preferred for treated animals.

What Do the Results Mean for Treatment?

A confirmed positive culture commits the household to combined systemic and topical therapy plus environmental decontamination, typically for six weeks or longer. Guidelines from the World Association for Veterinary Dermatology emphasize that topical therapy alone is rarely adequate for generalized disease, and systemic therapy alone leaves the animal infectious to housemates and people.

Systemic options in dogs include itraconazole, commonly dosed at 5 mg/kg once daily, and terbinafine at 30 to 40 mg/kg once daily, both prescription decisions made by the attending veterinarian based on species, liver function, and concurrent medications. Griseofulvin is now used less often owing to bone marrow and gastrointestinal effects.
