What Is Skin Cytology and Why Does the Stain Matter?
Skin cytology is the microscopic examination of cells and organisms collected from the skin surface, and the stain applied to that sample determines which structures become visible. A slide that looks empty under one stain can be diagnostic under another, which is why staining method is not a technical footnote but a diagnostic decision.
Cytology earns its place because of yield relative to cost. Published dermatology audits report that in-clinic cytology changes the treatment plan in a substantial share of itchy-dog consultations, most often by revealing yeast or bacterial overgrowth that was not clinically obvious. The test costs minutes and pennies, and it answers the question that governs every subsequent decision: is there an infection here, and if so, what kind.
Stains work by exploiting the chemistry of cell components. Romanowsky-type stains such as Diff-Quik combine an acidic dye (eosin) that binds basic proteins in cytoplasm and a basic dye (methylene blue or azure) that binds acidic nucleic acids in nuclei and in bacterial and fungal cell walls. That is why organisms appear deep purple-blue against a pink cellular background. Gram stain works on a different principle, retaining crystal violet inside the thick peptidoglycan wall of gram-positive organisms while gram-negative organisms lose it and take up the counterstain.
The practical implication is that a single stain answers a single class of question. Diff-Quik tells you whether organisms and inflammatory cells are present. Gram stain tells you what class of bacteria you are dealing with. Neither tells you the species or the antibiotic susceptibility, which is what culture is for.
What Are the Main Cytology Stains Used on Dog Skin?
Four staining approaches cover essentially all routine and referral canine dermatology cytology.
Diff-Quik (modified Wright-Giemsa) is the standard in-clinic stain. It is a three-jar system: a methanol fixative, an eosinophilic solution, and a basophilic solution. Total contact time is roughly 5 to 10 dips per jar, and the slide is ready in under a minute. It reveals cocci, rods, Malassezia yeast with their characteristic broad-based budding peanut shape, neutrophils, eosinophils, macrophages, and acantholytic keratinocytes.
Gram stain is used when rods are seen on Diff-Quik or when a pyoderma has failed empirical therapy. Gram-positive cocci stain purple and are usually Staphylococcus pseudintermedius. Gram-negative rods stain pink and raise the possibility of Pseudomonas, Proteus, or E. coli, organisms with very different antibiotic profiles.
New methylene blue is a rapid wet-mount stain useful for a quick look at nuclear detail and for confirming yeast, though it does not preserve as well as a dried, fixed preparation.
Special stains are laboratory-based. Periodic acid-Schiff (PAS) and Gomori methenamine silver highlight fungal cell walls in tissue sections and are used when dermatophytosis or deep fungal disease is suspected. Acid-fast staining identifies mycobacteria in nodular disease. Eosinophils: prominent eosinophils shift suspicion toward parasites, allergy, or eosinophilic dermatitis rather than bacterial infection.
Acantholytic keratinocytes: rounded, deeply basophilic keratinocytes floating free in a sea of neutrophils, without visible bacteria, are the classic cytological hint of pemphigus foliaceus and prompt biopsy rather than more antibiotics.
The pattern that most often surprises owners is inflammation with no organisms at all. That result is not a failed test. It is a meaningful negative that redirects the investigation from antimicrobials toward allergy management, and it is exactly the situation in which repeated courses of antibiotics do harm without benefit.
Comfort Care While the Diagnostic Workup Proceeds

Getting the Most From Every Cytology Sample
A few habits substantially improve diagnostic yield, and owners can influence several of them.
Do not bathe the dog for 48 hours before the appointment. Shampooing, especially with an antiseptic product, strips surface organisms and can convert a diagnostic sample into a normal one. The same applies to topical sprays and wipes applied to the area being sampled.
Sample before starting any antimicrobial. Even 48 to 72 hours of an antibiotic or antifungal can reduce organism numbers below the visual threshold while leaving the dog just as symptomatic.
